Guidance for Every Stage of End-of-Life

Six Stages, Six Seasons

We understand that circumstances around transitions can be overwhelming. Below is a simple framework of the six most common stages of the end-of-life journey. It's important to know that stages are non-linear - patients' conditions may fluctuate on this timeline. One of these stages might correspond more closely to what you're experiencing. To find providers in your county, click on the stage of end-of-life that best corresponds to your current situation.
